First Visit

Your first visit to our office will be in the form of a consultation. The physician will meet with you to discuss your gastrointestinal issues, medical and social history, and to perform a physical examination. They will then discuss with you a treatment plan and course of action.

Please Bring The Following Items With You To Your Visit

  • Most current insurance card(s)
  • Driver’s license or photo identification
  • Co-payment in the form of cash, check or credit card
  • A complete listing of your medication, including dosages
  • If you have seen a Gastroenterologist in the past and/or have had prior diagnostic testing done CT scan, x-ray, lab work, etc.), please have those records forwarded to us in advance or bring with you to your appointment.
  • Ensure that our office has received a referral, if necessary, in advance of your appointment.

Our office participates in many insurance plans. If your insurance plan is not listed, please call your insurance company to confirm participation.

Allied Digestive Health has partnered with HealthMark Group to ensure the accurate and timely completion of medical record requests.

How to Sign in

Once you enter your email, you will receive an email with HealthMark to log in (no username or password required!). Click on the “submit request” button and follow the prompts from there. You will receive an email as soon as your records are available for download.

FAQ

Do I have to remember another username and password?

Nope! HealthMark’s Request Manager uses email verification and secure links to get you your records quickly, efficiently and securely – and without yet another username and password to remember!

How long does it take to process requests?

Most record requests are processed within 8 business hours. So, one or two business days after you submit your request, your records will be delivered electronically – right to your inbox.

Is there a cost associated with obtaining my medical records?

HealthMark Group fulfills all requests for personal copies at no charge. If paper copies are requested a fee to cover delivery costs will be charged and must be paid before the records will be shipped.

When you call our office either ask or press the telephone prompts for the prescriptions department. You may speak to a live person or leave a message. Be ready to give your full name, date of birth, your phone #, medication needed and your pharmacy name and number. Please take note; all refills need to be evaluated by your doctor or a nurse practitioner for approval. If you have not seen your doctor in over a year, an office visit will be required prior to refilling your medication.

Call our office to obtain your results, whether you are inquiring about blood work, radiology testing, procedures and/or biopsies. Please realize results may take up to two weeks for a final report.

Colon cancer screening is best pOur office staff will give you detailed instructions at the time of you booking your procedure, however, here are some bullet points.
  • Procedures are done either at our freestanding Ambulatory Surgery Center; Advanced Endoscopy Surgical Center, Monmouth Medical Center or Jersey Shore University Medical Center.
  • Timeframe needed on the day of your procedure is approximately 2 hours from start to finish.
  • You will need to have a responsible adult to accompany you home. You will not be able to drive, walk or take a taxi on your own.
  • The responsible adult that accompanies you does not have to stay, you may be dropped off and they can return later within 2 hrs or someone can call them to pick you up if less time.
  • After your procedure, your doctor will speak to you regarding what to expect after your procedure, when to resume medications, and discuss any results (however, if a biopsy is done, this will take a few days for results).
